Advance Directives

If you ever lose the ability to make decisions for yourself, advance directives outline your wishes for your physician and family in regards to your health care.

UNC Health Rex provides you with a complete advance directives packet that can be downloaded and completed before you arrive for your procedure/treatment.

In North Carolina, some types of advance directives include:

  • A living will [PDF] informs UNC Health Rex and your family of your health care decisions should you become unable to do so.

  • Advance instructions provide instructions and preferences regarding mental health treatment.
