Medical Records

My UNC Chart is a secure online patient portal that allows UNC Health Care patients to see portions of their electronic medical record as they receive care from UNC Providers. Patients can request an account at myuncchart.org to gain access to selected test results, appointment information and summaries of clinic visits and hospital admissions at no charge.

Additional medical records can be requested from Health Information Management. We are open 8:30 AM- 4:30 PM, Monday- Friday.

Requesting your medical records

1. Fill in your Authorization form. (HIM #1409-s).

The authorization form can be obtained from any UNC Hospitals, UNC Hillsborough Campus or Chatham Hospital, or you can download a copy from the link.

Sign and return your completed form via:

Receiving your medical records

For questions regarding your medical record release, please call (919) 784-3158 between the hours of 8:30 a.m. and 4:30 p.m., Monday – Friday. Patients requesting records for their own personal use, fees will apply.

You can receive your records by:

  • Fax

For follow-up care, we can fax key medical record information to another care provider upon your request the same day or prior to your scheduled appointment (at no charge). We will not fax for non-patient care purposes.

  • Mail

Your records will be processed within 3-5 business days upon receipt of your completed form. (You will be invoiced upon receipt from HealthPort Copy Service.)

  • Pickup

You can pick up your records (upon notification that they are ready) at:

Our main location is conveniently located at UNC Health Rex, 4420 Lake Boone Trail, Raleigh, NC 27607. Simply check in at the main entrance and you will be directed to our office.

Amending your medical records

You may request an amendment of protected health information (PHI) about you by completing the Request for Correction/Amendment form and mailing or faxing it to the Health Information Management Department, Attn: Release of Medical Information:

UNC Health Care must act on your request for an amendment no later than 60 days after receipt of such a request. If UNC Health Care is unable to act on the amendment within the required 60-day time limit, UNC Health Care may extend the time for such action by no more than 30 days, provided that:

A UNC Health Rex Privacy Officer provides you with a written statement of the reasons for the delay, and the date by which action on the request will be completed; and

UNC Health Care may have only one such extension of time for action on a request for an amendment.

You will be notified in writing if your request was accepted or denied.

UNC Health Immunization Records (including COVID-19 Vaccination)

Your UNC Health immunization record can be found in My UNC Chart.  If you have My UNC Chart, you can login to access these records. If you do not have My UNC Chart and would like to activate your account, you may activate online at https://myuncchart.org/MyChart/accesscheck.asp or by calling UNC Health Outpatient Access Center at (888) 996-2767. You may also contact UNC Health Medical Records at 984-974-3226 for a copy of your Epic@UNC immunization record, but please note that they will not be distributing CDC Vaccination Cards.

You may also contact NC DHHS on their Help Desk number at (877) 873-6247 to get your vaccination records printout.
