Referring a Patient

• Use CareLink, our electronic referral portal, and get secure, read-only access to Epic electronic medical records at your convenience.

TAVR Referrals

To request a patient evaluation for transcatheter aortic valve replacement (TAVR), you may:

• Call 919-784-1321 or email heartvalve@unchealth.unc.edu.
• Download the REX Heart Valve Center Patient Referral Request Form [PDF] and REX Heart Valve Center Record Release Form [PDF] and fax or mail them.

TAVR Patient Criteria

Consider referring your aortic stenosis patients for TAVR if they are:

• High risk
• Symptomatic
• Older than 80
• Experiencing comorbidities

If your patients are candidates for surgery, refer them for conventional aortic valve replacement.
